Blue Line Health & Wellness Group proudly supports Canadian veterans through comprehensive mental health disability assessments for Veterans Affairs Canada (VAC).

Our assessment process utilizes a collaborative, multidisciplinary approach involving a Registered Social Worker, Registered Psychotherapist, and Nurse Practitioner to ensure a thorough and evidence-based evaluation of your mental health concerns and their impact on daily functioning.

As part of the assessment process, our Registered Social Worker or Registered Psychotherapist conducts a detailed psychosocial assessment, reviews military and service history, administers and interprets standardized psychological screening measures, and prepares a comprehensive clinical report outlining presenting concerns, functional limitations, symptom severity, and treatment recommendations.

Our Nurse Practitioner completes a medical review, provides the diagnostic opinion, and prepares the required VAC PEN 6248 Medical Questionnaire. This fulfills Veterans Affairs Canada’s requirement for assessment and documentation by a qualified medical practitioner.
