What does an integrated care approach mean?

Integrated care looks at the whole person rather than treating each concern in isolation. It may bring together evidence-based mental-health support, trauma-informed care and appropriate wellness services within a coordinated plan.

 

Do I need to use every service offered?

No. Your care plan is based on your needs, goals, comfort level and professional recommendations. Some clients may benefit from one service, while others may use a combination of supports.

How is my care plan created?

The process begins with understanding your concerns, history, current stressors and goals. From there, the team can help identify suitable services and adjust the plan as your needs change.

Is integrated care suitable for First Responders and veterans?

Yes. Blue Line’s approach is designed with an understanding of trauma exposure, operational stress, demanding work environments and the unique pressures experienced by First Responders, veterans and their families.

Are wellness services a replacement for psychotherapy or medical care?

No. Supportive wellness services are intended to complement appropriate professional care, not replace psychotherapy, medical treatment or emergency services. Recommendations depend on each client’s circumstances and assessment.

Will my information remain confidential?

Client privacy and confidentiality are central to the care experience. Any coordination between professionals is handled according to applicable privacy requirements, informed consent and the clinic’s policies.

How do I get started?

You can contact the Blue Line team or book an appointment online. The clinic can help you understand available services and determine an appropriate next step based on your needs.

Veterans Affairs Canada (VAC) Disability Assessments

Blue Line Health & Wellness Group proudly supports Canadian veterans through comprehensive mental health disability assessments for Veterans Affairs Canada (VAC).

Our assessment process utilizes a collaborative, multidisciplinary approach involving a Registered Social Worker, Registered Psychotherapist, and Nurse Practitioner to ensure a thorough and evidence-based evaluation of your mental health concerns and their impact on daily functioning.

As part of the assessment process, our Registered Social Worker or Registered Psychotherapist conducts a detailed psychosocial assessment, reviews military and service history, administers and interprets standardized psychological screening measures, and prepares a comprehensive clinical report outlining presenting concerns, functional limitations, symptom severity, and treatment recommendations.

Our Nurse Practitioner completes a medical review, provides the diagnostic opinion, and prepares the required VAC PEN 6248 Medical Questionnaire. This fulfills Veterans Affairs Canada’s requirement for assessment and documentation by a qualified medical practitioner.

Together, our clinical team produces a complete assessment package that can be submitted to Veterans Affairs Canada in support of a disability benefit application or reassessme

Ongoing Mental Health Treatment

In addition to disability assessments, Blue Line Health & Wellness Group offers ongoing mental health treatment for veterans and their families through both virtual and in-person appointments.

Our team of Registered Psychotherapists and Registered Social Workers provides trauma-informed therapy tailored to the unique experiences of military service, operational stress injuries, PTSD, anxiety, depression, grief, relationship challenges, transition to civilian life, and other service-related concerns.

For eligible veterans, treatment services can be direct billed through Medavie Blue Cross, reducing financial barriers and simplifying access to care. Our administrative team is available to assist with eligibility verification and the authorization process to help ensure a smooth and supportive experience.
